Application Forms 2017 - 2018 (2017-03-31) (2017-04-30)

Please find attached an application form for season 2017 - 2018. Clubs are asked to complete the application form in full. TBA answers will see the form returned. The closing date for applications is 30th April 2017.
Any new clubs applying must forward a cheque for £250.00 (per team) and any existing club wishing to add an extra team must also forward the same amount (£250). All current clubs and teams have NOTHING to pay until the AGM in June for the season's fees.
All completed forms should be emailed please to : firstname.lastname@example.org
ALL application forms will be acknowledged by the office.
Any new clubs wishing to apply will be subject to an interview with the Management Committee and these meetings will take place early in May so that you will have an early indication of whether you have been successful.

We are still happy to accept paper based applications, however we would also welcome you to use the new electronic application form which may save many of you some time.
